Hey all- dog mom of a recently diagnosed epileptic 6ish year old Pittie. Initially started keppra with good control for about 6 weeks. Then added phenobarb. Made it about 3 weeks without a seizure. Increased the keppra and have added in CBD. Due to my work schedule (12 hour shifts; coming home halfway between to let them out), I’m not able to keep an eye on her and/or observe and document the seizures. My anxiety is through the roof and I’m just having an all around hard time. Her vet said it was likely idiopathic; however, recommended neuro consult if we can’t get good control. At this point, I’m terrified it’s something terminal or progressive because of the increase in seizure/seizure like behavior. Have you all had to play with meds? What has been your experience in getting things under control? I am willing to get a consult but would like to exhaust all other options due to financial concerns.
Thanks all for this subthread. It has been so helpful in quelling some of my worries and fears.

So some background story... my wife and I wanted to be closer to family so we decided to move. Since the city we're moving to is 1.5 hours (no traffic) away from my previous company, I decided to take the opportunity to quit and do a career switch and joined a coding boot camp because I've always wanted to pursue a career in tech. I just graduated back in February and definitely having a hard time finding my first developer position much like everyone else.
I've now been presented with a unique opportunity at my previous company to be a summer web developer intern. It is 10 weeks for $20/hr and it's on-site... so not looking forward to the commute. I've seen a lot of posts of people with years of industry experience or internships still not able to find a job in this current economy. This gets me worried if I'll be suffering for nothing if I take this internship.
Would it be worth it to suck it up for 10 weeks and pursue this internship in hopes of it improving my chances later, or not pursue and continue focusing on finding better opportunities?

I recently started ACT but I'm struggling to... relate? to the homework/worksheets. E.g. the text stating there's virtually no physical threat left in the modern world and the threats we perceive are purely psychological. As a survivor of DV and SA I obviously disagree.
I tried to talk about this with the ACT therapist but found it hard to explain because I was sort of beating around the bush. He asked me if I could give an example of why I didn't relate to that particular piece of text and I ended up blurting out the SA. Then I got asked about my boyfriend and the tone of it made me feel like he as trying to conclude that the SA wasn't impactful because I have a boyfriend now. I told him that I talked about it with my boyfriend. Therapist asked whether he then makes sure I'm okay and doesn't cross my boundaries and I answered yes.
I'm on the waiting list for some form of trauma therapy (to be determined) because there is a number of negative events/memories that's so strong that I can barely even cope with thinking or talking about them. And this ACT is meant to keep me from crisis while I'm on the waiting list, but it's already making me dig up memories that are basically not safe to get into... one activates the other and they're all related to a shitty, unsafe, violent childhood that could've been prevented if even 1 person stepped up. I tend to get in crisis when they get activated/triggered strongly, which is the opposite of the therapy's goal.
[Disturbing nightmare description ahead, only in this paragraph] I've been feeling out of sorts since the appointment at the beginning of this week and last night I had a nightmare about my GP forcing herself into my room and saying she doesn't believe me and starting to undress me.
Woke up panicked and with a headache. Usually my nightmares come in series, I'll wake up with them for like a week straight. I've had so much chronic stress that I get shitty physical symptoms (even saw a psychosomatic physiotherapist after being checked for basically everything) and I've been out of breath and headache-y recently already. Now I just woke up out of breath and with a headache, instead of it starting later in the day. (Please don't tell me what physical disease I could have. I've been very well checked over for years and been taken very seriously)
This ACT feels wrong to go into when the therapist doesn't know anything about my trauma and also because I start to dissociate a bit (foggy mind, can't think, can't remember what the other person said) so the appointment got very blurry and confusing towards the end. This therapy was supposed to help me stabilise but I feel actively worse than before the appointment and I have had 0 tips (ever, aside from 'distraction!!!') on how to cope with with everything coming back

STUDY QUESTION OR PURPOSE OF THE TRIAL
To establish safe and efficacious dose of Burton’s tyrosine kinase (BTK) inhibitor evobrutinib (previously called M2951) in people with relapsing multiple sclerosis (MS).
BACKGROUND
  • Role of Burton’s tyrosine kinase signaling in MS is summarized here, here. BTK inhibitors may have the advantage over existing MS disease modifying therapies since BTK inhibitors are CNS penetrant and, thus, could target both peripheral immune compartment (blood, lymph tissues) as well as CNS compartment (microglia) (here).
  • Evobrutinib is a selective, oral, BTK inhibitor. It blocks B-cell activation and cytokine release and inhibits the activation, differentiation, and polarization of proinflammatory M1 macrophages and their release of cytokines in vitro.
  • This was a phase 2, randomized, double-blind, placebo-controlled trial to establish safe and efficacious dose of evobrutinib in people with relapsing MS (pwRMS).
WHERE AND HOW
  • The study enrolled 267 pwRMS (aged 18-65 years; EDSS up to 6 at screening) at 56 sites across Europe and Russia. The study included both people with relapsing-remitting MS (228, 87% of total) and secondary progressive MS with superimposed relapses (33, 13% of total).
  • The study participants were randomized 1:1:1:1 to evobrutinib 25 mg once daily, 75 mg once daily, 75 mg twice daily, placebo, or open-label dimethyl fumarate (DMF). DMF was used as a reference drug in this trial.
  • The length duration of the trial was 52 weeks including 24-week placebo-controlled phase followed by a 24-week blinded-extension phase and a 12-week safety follow up. At the start of the blinded extension phase, the placebo group crossover to evobrutinib 25 mg daily; all other groups continued with the originally assigned treatment/doses. Exposure to study drug was 48 weeks across evobrutinib groups and 24 weeks for placebo group.
  • MRI scans were performed every 4 weeks starting at week 12; EDSS at weeks 12, 24, 36, and 48; safety through week 52, the end of the study.
  • The primary endpoint was the total (cumulative) number of gadolinium (Gd)-enhancing lesions identified on T1-weighted MRI at weeks 12, 16, 20, and 24. The reading at week 12 was considered baseline.
  • Key secondary endpoints were the annualized relapse rate, based on qualified relapses; qualified relapse-free status; change from baseline in the EDSS score at week 24; and safety.
  • The study was powered at 85% to detect a 90% lower number of Gd-enhancing lesions, if each evobrutinib group includes 50 participants assuming a 12% dropout rate.
RESULTS
  • Baseline characteristics were similar across all groups with mean age of 42 years; 69% female; median time from onset of MS symptoms of ~7.5 years; mean EDSS score of ~3.3; and mean (SD) number of Gd-enhancing lesions at baseline of 1.54 (4.37). Each study group enrolled 52 to 54 participants and 47 to 52 completed the 24-week placebo-controlled phase.
  • Primary endpoint: The mean (SD) total number of Gd-enhancing lesions decreased in a dose-dependent manner from week 12 to week 24 in evobrutinib treatment groups versus placebo: 4.06 (8.02), 1.69 (4.69), and 1.15 (3.70) in the evobrutinib 25 mg daily, 75 mg daily, and 75 mg twice daily groups, respectively, versus 3.85 (5.44) in the placebo group.
  • The baseline adjusted rate ratios for the total number of lesions over time also decreased in a dose-dependent manner from week 12 to week 24 in evobrutinib treatment groups versus placebo: 1.45, 0.30, 0.44 in the evobrutinib 25 mg daily, 75 mg daily, and 75 mg twice daily groups, respectively. After adjustment for multiple comparisons, the p values were significant for evobrutinib 75 mg daily (p = 0.005) and 75 mg twice daily (p = 0.06) groups.

  • Secondary endpoints: the number of relapses from baseline to weeks were lower in 75-mg evobrutinib groups (3 relapses in 75 mg once daily; 2 in 75 mg twice daily) versus 9 relapses in placebo and 13 in 25-mg group. The corresponding unadjusted annualized relapse rates were also lower in 75-mg evobrutinib groups (0.13 in 75 mg once daily; 0.18 in 75 mg twice daily) versus 0.37 relapses in placebo and 0.57 in 25-mg group. (Differences versus placebo was not significant for any evobrutinib group).
  • Safety: The most common adverse events of any grade were nasopharyngitis and increases in levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), and lipase. The increases in ALT and AST did not reach the Hy’s law DILI criteria.
CONCLUSIONS
  • The 75 mg once daily dose significantly reduced the total number of Gd-enhancing lesions on T1-weighted MRI, measured at weeks 12 through 24. This dose was considered safe and efficacious for further study of evobrutinib in larger MS trials.
  • Currently following phase 3 evobrutinib trials are ongoing in relapsing MS: NCT04338022 (evolution RMS 1 trial) and NCT04338061 (evolution RMS trial 2) . Sponsor: Merck Healthcare KGaA, Darmstadt, Germany / EMD Serono Research & Development Institute, Inc.
DISCUSSION
  • Compared to the tolebrutinib phase 2 trial (here), the participants in the evobrutinib trial were older, with longer disease duration, and fewer relapses, and relatively high baseline EDSS values.

In celebration of CLOT’s 20th anniversary, Founder and Creative Director Edison Chen and long-time collaborator and friend Hiroshi Fujiwara have teamed up with Nike to produce a new collaboration sneaker: the Dunk Low x CLOT x Fragment Design. Inspired by Hiroshi Fujiwara’s favorite CLOT shoe of the last two decades—the Air Force 1 x CLOT, also known as the “White Silk” from 2018—the Dunk Low x CLOT x Fragment Design features an all-white silk-inspired upper patterned with CLOT’s iconic Silk Royale, set on a cream-white opaque sole for a clean look. The tongue, lace stay, and heel tab are rendered in black, resulting in the striking black and white colorway inspired by pandas, which Fujiwara fondly recalled visiting with Chen at the Chengdu reservation. The insole, meanwhile, features the CLOT20 logo, marking this a special release. Each pair of Dunk Low x CLOT x Fragment Design comes with black and blue laces for styling purposes.

“Contrary to what some may think, our conversations rarely revolve around work. Most of the projects that come from me and Hiroshi, they’re very organic. This project is very rare. Hello, Accounting peeps!
I just received my Law on Sales FG, and it was beyond the retention grade 💀. I still have to retake Parcor Accounting (and prolly Cost Accounting and Intermediate Accounting WELP).
And like, the schedule for summer classes overwhelmed me since it'll be conducted everyday of the week.
I'm also starting to lose interest in becoming a CPA already, so I started looking into different alternatives. I initially thought of BS Management Accounting, but I wondered about BS Accounting Information Systems. I mean, it's kinda good because it's a combination of Accounting and Tech. And knowing how Fintech is starting to prosper, BS AIS sounds like a good choice!
But during ftf, I barely heard of BS AIS HAHSBAHAHAB it's usually BSA and BSMA lang sa department 😭 are there still BS AIS students now???

Friends but never anything more. We had real love and admiration for each other— or I did at least, but he had just gotten out of a toxic ass relationship and was giving me mixed signals. Holding hands, kissing me, introducing me to friends, etc.,.
We ended on weird/bad terms. When confronting his behaviors he was extremely defensive and insensitive. Not only did he say he didn’t reciprocate those feelings but that he didn’t care if we were friends or not.
Unfortunately, we’re both petty and stubborn. So I left without saying a word and I think he expected me to reach back out to him. I never did because I’d be an idiot to. We unfollowed each other and assumed we were under the law of “no contact”. However, it doesn’t mean I don’t still care about him.
We see each other out sometimes but we don’t speak. There’s tension for sure. Last week, I found out that he had recently blocked me on insta.
My question is this: Why would he block me if he claimed to not give a shit?
