Tl;dr - got new insurance after job loss (USA), pharmacy submitted claims to old insurance. Is there any chance I’m going to have to pay that money back?
So I lost my job about a month ago. The employer didn’t do any kind of off-boarding, just sent a DM saying “today’s your last day.” I naturally assumed that meant my insurance ended immediately, so I bought a plan from the marketplace that day as I have quite a few prescriptions and wanted to make sure those were covered. Used my pharmacy’s app to upload an image of my new card via their “update your insurance” as soon as I got it.
Fast forward to this week where I refill all of my prescriptions and get an email from the old insurance a couple of days later about “viewing my claims” even though last I checked I no longer have access to that portal.
Checked my new insurance and no claims appeared there. Called the pharmacy this morning and they confirmed that they indeed had submitted the claims to the same company they had been. Updated what was on file, but now I’m worried I might have to pay back the other insurance company at some point. Is that a thing? Do I need to worry about that at all? Just trying to figure out if I should get in front of this and start making phone calls rather than wait for an unexpected bill.

I was diagnosed almost a year ago, and have been trying to get my medicine squared away since then. My doctor is really nice, about my age, and she also has adhd so I find it easy to talk to her. What I'm wondering is if maybe I should go to the dr that my new insurance has listed on my card, or if I should just stick it out with the current providers and keep trying instead of starting from scratch. The good thing about the new insurance is that i can now get some sort of therapy. The only thing I had access to with the old insurance was not what I need....basically just drug treatment and juvenile psych stuff.
I have a couple concerns about switching though. I already feel comfortable with the people at the current office, and sort of maybe have the ball rolling, so I'm hesitant to start from scratch. I also really like that the doctor I have is a woman in her 30s, not a man in his 60s. This post is for "Dyggoran Revival Project" ( Pronounced: Die-Ore-Rahn ) / DRP. DRP is my first, oldest and primary world building project which I have been working on since 2014. DRP began as a failed RPG Minecraft Server that was developed by myself and a few former friends. Due to a hard-drive crash in 2016, the only copy of the server's world was deleted before it could become a full-fledged server and after some time, I eventually decided to reboot "Dyggoran" outside of Minecraft as "DRP" and the rest in history.
Although DRP is predominately original, it is and always has been partially fan fiction-based. Aside from featuring a great many references too and elements from Minecraft, it also incorporates characters, details and elements from other works of fiction. The fan fiction elements of DRP make up an estimated 25% of the project with the remaining 75% being comprised of original work.

Jonathan and Sherwin, the main characters of the 2017 LGBTQIA+ short film "In a Heartbeat" function as the primary protagonists of DRP's first and introductory story which is a rewrite of The Shadow of Israphel. As the protagonists, Jonathan and Sherwin replace "Honeydew" and "Xephos", the Minecraft Avatar's of Simon Lane and Lewis Brindley.
For reasons that should be obvious, The Jonathan and Sherwin of DRP deviate heavily from their original versions. They posses the same names and designs but their backstories, personalities and other character attributes are almost entirely original on account of the fact that Jonathan and Sherwin did not speak in IaHB and had no backstory given on them.
In DRP, the story of Jonathan and Sherwin actually begins 124 years before The Shadow of Israphel.

JONATHAN
In DRP, Jonathan's full name is Jonathan Reyes. He was born 24 years before the "Nether Crisis" ( 24 B.C ) in the now defunct Kingdom of Panachia. Jonathan's father was the 2nd Chairman and CEO of Julien Laboratories / J-Labs, a former Panachian Mega-Corporation that is based on Aperture Science ( Portal/Half-Life ).
Jonathan grew up very privileged, aside from the fact that his father possessed a net-worth of over 100 Billion from the immense profits of J-Labs. His grandfather and the "founder" of J-Labs had been knighted by the King of Panachia, thus making the Reyes family a legal noble house. Possessing the titles of SiSer, the Reyes were minor nobility but commanded overwhelming power with J-Labs.
Although Jonathan enjoyed the benefits of substantial wealth and minor nobility. He had an arguably unpleasent home-life. His relationship with his father was distant, similar to typical depictions of the relationship between Norman and Harry Osborn in Spiderman. Worsened somewhat by the fact that Jonathan wasn't even the heir to J-Labs, that was Jonathan's older brother by 2 years Arthur Reyes.
Like in IAHB, Jonathan attended a school known as "Newgate Academy" which in DRP, is a private school. Jonathan was a highly accomplished student and was part of the school's baseball team where he was recognized as the best player.

SHERWIN
In DRP, Sherwin is Dwarven. Like Jonathan, he was born in 24. B.C in the Kingdom of Panachia. The Dwarves of DRP are mononymous, this means that they don't utilize surnames and only posses one name. Furthermore, It's also important to note that "Sherwin" is not a Dwarven name. In DRP, it is actually a "Panachianized" form of his birth name "Shtravraz" which is rather unpleasant looking and sounding and too difficult for non-Dwarves to pronounce.
Being adapted to underground life, the Dwarves find the surface to be uncomfortably spacious and they don't typically live or work on the surface unless they are contracted by Human and Elven companies.
Sherwin lived on the surface because his family was employed by a local "Super-Mine", Super-Mines are exceptionally large and deep mines that are specially built by Dwarven engineers and miners to achieve maximum resource output. Super Mines can operate for decades and this combined with the substantial pay Dwarven miners receive on account of being unionized, allowed Sherwin to gain an education at a private school such as Newgate Academy.

IN A HEARTBEAT AND AFTERWARDS
In DRP, Jonathan and Sherwin met in 10 B.C in the same way they did in IaHB save for the fact that no one had a sentient, anthropomorphized heart popping out of their chest. I've always held the opinion that the exact events of IaHB are a metaphorical depiction of what actually happened and this is the case in DRP. Whatever the case, Jonathan returned Sherwin's feelings and the two of them started dating.
Jonathan and Sherwin's relationship was kept secret as homosexuality was frowned upon in Dyggorani civilization by 10 B.C. To most people, Jonathan and Sherwin were nothing more than good friends.
In 6 B.C, Jonathan's father and Arthur were assassinated. Following a socially suffocating funeral in which Jonathan mourned only his brother, he became Chairman and CEO of J-Labs at 18 years of age. Jonathan decided to enact multiple positive reforms to the company which aimed to greatly improve employee health and safety, expand employee benefits and encourage and enable unionization, Jonathan also disbanded several divisions of J-Labs such as the highly lucrative arms division and relocated the employees of said disbanded divisions to other departments instead of terminating their employment. Lastly, Jonathan cut his own pay by more than 50% in order to enact a minimum salary plan for the employee population of J-Labs.
Jonathan's reforms caused a sharp increase in productivity, retention and employee happiness, however, it diminished profits by a rather significant amount. The J-Labs Board of Directors was not happy about this and began conspiring to remove Jonathan from power.
In 5 B.C, the J-Labs Board attempted to stage a coup against Jonathan, using the J-Labs Corporate Security Force ( the company's private army ) to seize control of J-Labs properties. The coup ultimately failed due to the support of the Panachian unions. During the coup, however. Sherwin was grievously injured by a crossbolt that had been coated in an incurable poison, a bio-weapon that had been developed by the now defunct J-Labs arms division. Unwilling to let his boyfriend die, Jonathan put Sherwin in a specialized cryo-stasis pod until a cure could be found.
Over the course of the next 5 years, Jonathan suffered from severe depression and withdrew from public life, relegating most of his responsibilities as CEO to his secretary and only emerging when absolutely necessary.

THE NETHER CRISIS
The previously mentioned Nether Crisis of 0 B.C / 0 A.C was a cataclysmic event that caused a soft to moderate collapse of Dyggorani civilization.
"The Nether" is an alternate plane of existence that has been known to the Dyggorani for millennia as the source of "dark magic". As a dimension, the Nether is overtly hostile to all foreign life and matter and even though the dark magic sourced from the Nether is extraordinarily powerful, prolonged use of it can cause severe biological and mental mutations that can turn even the greatest mages into eldritch horrors.
At some point after the J-Labs Coup d'état attempt, Jonathan approached Black Mountain, another scientific mega-corporation that had historically rivaled J-Labs. BM was working on a large-scale project known as the "Netheric Energy Network" or NEN which, via technology that it partially stole from J-Labs about a decade or so prior, was supposed to power all of Panachia with Netheric Energy, turning the nation into an energy self-sufficient superpower.
Eager to put their rivalry aside and convinced that the power of the Nether could somehow save Sherwin, Jonathan negotiated a contract with Black Mountain in which the two companies would work together to complete the NEN which would be managed by a joint-venture subsidiary of both J-Labs and Black Mountain known as Encom Systems.
For reasons that have yet to be determined, the NEN suffered a catastrophic failure on the day of its intended activation in 0 B.C/A.C, opening a massive aerial Nether Portal that unleashed the horrors of the Nether onto Dyggoran, causing massive amounts of death and destruction. Jonathan was present at the activation ceremony along with BM's CEO and the King of Panachia. All of them, Jonathan included were presumingly killed in the disaster.
The Nether Crisis only lasted around 27½ hours but it both dramatically and violently altered Dyggorani civilization.

THE SHADOW OF ISRAPHEL
The Shadow of Israphel begins 100 years after the Nether Crisis ( 100 A.C ) in the J-Labs Science Center, a massive corporate campus in Western Panachia that functioned as the primary research and development center of J-Labs. The Science Center was abandoned during the Nether Crisis and never re-occupied.
After 100 years of neglect, the Science Center's power plant has gone critical and is in imminent danger of meltdown. As a result, the Science Center's emergency evacuation procedures are activated, this awakens Sherwin and a clone of Jonathan from a little over 1 century of cryo-sleep.
Both Sherwin and the clone of Jonathan have cryo-sleep induced amnesia. They have no idea who they are or where they are but they are able to determine a faction of their identities from the damaged stickers on their cryo-pods.After escaping from the Science Center an hour or so before meltdown, Jonathan and Sherwin survive in the winter snow-blanketed wilderness of Western Dyggoran for about 20 days before a mysterious entity by the name of Israphel causes them to have a run-in with an old man named Obadiah. The story of SOI begins from that point forward and the rest is history.

The reason why Julien Laboratories uses the name "Julien" instead of "Reyes" is because the company actually began as a small-town lab that was founded and operated by a scientist named Dr. Robert Julien. tl:dr; An extensive post on prior authorizations, copays, coupons and getting Wegovy covered. Recommended reading if you haven't started the process yet, want to get Wegovy covered on your insurance, or if your prior auth request was rejected. I talk about copays, the prior auth process for first doses and for dose increases, and what kinds of things you might need to qualify to get Wegovy covered by your plan.

I want to preface this post by stating that I am a pharmacist, but not currently working as a pharmacist. I have loads of experience dealing with insurance companies and the prescription billing process. I worked in a retail pharmacy for 25 years. I have also worked for an insurance company as a pharmacist. I started Wegovy 3 weeks ago and I had to go through a 6 week process of cooperating with my doctor's office and the insurance company to get it covered, and I wanted to share the process I went through and some tips for anyone trying to get it covered for themselves.
I cannot promise that following what I have done will work for you, each plan does it's own thing and make their own decisions.
I will not be discussing supply issues here, this is not the post for that.
I started with visiting the website and looking at what indications the med was for. Indications mean which medical conditions and symptoms has the drug been approved by the FDA to treat. According to the Wegovy website Wegovy.com , " WEGOVY® (semaglutide) injection 2.4 mg is an injectable prescription medicine that may help adults and children aged ≥12 years with obesity (BMI ≥30 for adults, BMI ≥ 95th percentile for age and sex for children), or some adults with excess weight (BMI ≥27) (overweight) who also have weight-related medical problems to help them lose weight and keep it off. Wegovy® should be used with a reduced calorie meal plan and increased physical activity"

This information gave me clues as to what the insurance company is expecting for the basic qualifications in the prior authorization request:

1. BMI 30 or above, or BMI 27 or above with weight related health issues
2. Reduced calorie meal plan
3. Increased physical activity

There are more qualifications that each insurance company will require to make sure that the medication is deemed appropriate for you. I had the basic qualifications, so I felt like I had a good chance of getting it covered. I did some more research. I googled "Wegovy Prior Authorization Criteria" and found some links for some common insurance plans. I didn't find mine, so I checked out a few of the ones listed to see what other kinds of criteria my insurance might want. This pdf was helpful and I thought was a good example: Wegovy Caremark Criteria . This form gives their interpretation of what their specific conditions are for coverage, and if I met those, I felt I probably qualified under my own plan.
I talked with my doctor, she had done prior auths for Wegovy before so she knew a few of the things they wanted, so she took notes and had her assistant send in the prior authorization request with the following information:

1. Has the patient tried a structured diet (keto, vegan, DASH, etc) in the last 12 months? yes, Keto and other low carb. stopped due to cholesterol levels
2. Has the patient tried a behavioral weight loss plan (Weight watchers, Noom, etc) in the last 12 months? yes, Noom. It helped somewhat, but was too expensive for me.
3. How much exercise does the patient participate in per week? Gym membership? Walking 3 times per week for 30 minutes, no gym membership
4. What's the patient BMI? 36
5. Does the patient have any obesity related health conditions such as obstructive sleep apnea, dyslipidemia (high cholesterol), hypertension (high blood pressure), or type 2 diabetes? yes. apnea, dyslipidemia, hypertension.

The initial request was sent in to the insurance. According to them, the initial request was missing information. My plan requires the remaining information to be submitted to them by the provider within 24 hours of the initial request in order to process the authorization. Since it was after 5pm, they said they were not able to contact the office, so they sent a fax request for more information. Of course my office doesn't look at faxes as soon as they come in, they are busy seeing patients!
The 24 hour deadline came and went, and I received a rejection letter in the mail from the insurance plan. I called them for more information on the letter, to find out what information was missing and what needed to be done.
Here are the questions I asked and their answers (THESE ANSWERS ARE SPECIFIC TO MY PLAN, YOURS WILL PROBABLY BE DIFFERENT):

1. What information is missing from the original authorization request? A statement from the provider that the patient is not pregnant and patient has no contraindications to using the medication. (A contraindication is any reason why a person should not use the med, like having thyroid cancer or being allergic to the active ingredients)
2. How can the provider submit that missing information to you? An appeal needs to be filed. The provider needs to send a letter or another prior authorization request to us that includes the missing information.
3. Can I start the appeal process myself? You can start the appeal process, but we will still need information from the provider.
4. Does the form need to be faxed to you or sent to you online? The provider can submit either by fax or via an online prior authorization service. Either way works for us.
5. If my office faxes you, is there a specific form that they need to use? No, just their own standard office prior authorization form works.
6. Is there any other information or requirements that the original prior authorization request missed? No, just those 2 items.
7. How long does the authorization process take? After the appeal is received by our office, we have 28 days to review and give a determination.
8. How will I know if it has been approved? Do you contact me or the pharmacy? We will contact your doctor's office first, and you will receive a letter in the mail with the determination. You may also call us back anytime over the next couple of weeks to check the status. It will be given a due date, and we can let you know what date has been assigned once we have received the appeal.

Satisfied that I had gotten all of the answers I needed, I emailed the medical assistant at the office and requested that they send the additional information to my insurance and start the appeal. I emailed the questions and answers that I received during my phone call to give her all the information she might need to get the request submitted properly. I was SUPER NICE AND PATIENT with her, as I know she has loads to do and might not be able to get to it right away. I told her in the email that I appreciated all the time and effort she was giving to help me get this covered.
After a couple of days, I received an email from the assistant with a notification that the insurance company wanted me to sign a release form to allow the assistant to 'vouch' for me for the appeal.
The rest was just waiting and checking back with the insurance plan about once a week. It was approved after about 3 weeks of checking back.
SUCCESS! The approval was for the first strength, Wegovy 0.25mg weekly. The whole process was about 6 weeks because I didn't want to bug my medical assistant more than once a week.

Now the question is: do I need another prior authorization for a new dose? I called the insurance company again. Questions and answers (again, these are SPECIFIC TO MY PLAN) :

1. Do I need another prior authorization to go to the next dose for Wegovy? Yes. But the process is a little easier for the dose increases.
2. What is that process? Do they have to send all the original information again for the new dose? The provider submits a generic prior authorization request, but this time, the only real notes that need to be included are a statement to the effect of "Dosage increased per FDA recommendation".
3. I am expecting to have several dose increases. Does each prior auth have to be done separately, or can the provider request all the doses at once? First, I need to tell you that prior authorizations can be requested up to 30 days before the prescription is filled. Because of that, the provider can only request 2 doses at a time. If they use an online form, they would need to send the dose requests separately. If they fax us, they can just write in the comments or cover letter "Wegovy 0.5mg AND Wegovy 1mg".

My plan is to send these questions and answers to my medical assistant again, a few days before my next appointment, with a reminder to her that I will be asking my provider to increase to the next dosage, Wegovy 0.5mg, and expect to increase to the next strength 28 days after, and ask SUPER NICELY if she would please send that prior authorization request for the next 2 doses after my upcoming appointment so there's not a big delay in being able to received my next dose.

TIPS:

1. Figure all this out ahead of time so you aren't disappointed and/or waste a trip to the doctor or waste everyone's time if you don't meet the qualifications for your insurance plan. There are so many people involved in getting you this med, that if in the end you don't end up getting it, it's a huge waste of time and resources. Just think of all the people involved: the scheduler for the appointment, the med assistant, the provider, the person submitting the prior authorization request at the office, the pharmacy technician, the pharmacist, the prior authorization people at the insurance company, etc.
2. You can contact your insurance plan before you even talk to your doctor. Ask to speak to a pharmacy services representative, and if they are not able to answer your questions, ask to speak to a representative in the pharmacy prior authorization department. Ask all the questions listed above, write all the answers down and take them to your doctor if you qualify.
3. Make an agenda before your appointment. Write down all the answers to the qualification questions that you discovered by contacting your insurance company, and hand it to your assistant at the beginning of the appointment, so it's easy for your provider (or scribe) to just throw it in your office note at the appointment.
4. Pretend like your medical assistant or prior authorization specialist at your provider's office has no clue. What I mean is that they know how to submit the authorization, but there are so many insurance plans that there is no possible way for them to know each plans process and requirements for the authorization submission for each medication. A lot of times it's a trial and error thing. Send it and see if it 'sticks', if not, change it up and send it again. It's a heck of a lot easier on them (and more efficient for you) if you give them the requirements that you learned by calling your insurance ahead of time.
5. While you wait for a determination, calling and asking your pharmacist "is it covered yet" is NOT helpful and it actually costs the pharmacy money! Each time a claim is submitted or attempted - sent to your insurance plan through the computer, there is a charge for the transmission. It's best to keep in contact with your insurance plan and call them daily or weekly, or visit your plan's online portal to see if they have updated the authorization. They also get cranky when you keep calling - they are busy as heck!
6. Sometimes even after the prior authorization has been approved, it might still reject at the pharmacy. When claims are processed through that have dollar amounts higher than $1000, they often will get 'stuck' at the processor. I believe it's a code 78 rejection. The pharmacy technician will need to contact the help desk to get this resolved (phone number should be provided in the claim rejection information).
7. Be patient.

COPAYS
Quick note on copays. When you contact your insurance plan about the requirements for coverage, ask at that time what YOUR copay for the med will be, once it's approved. A lot of things determine what that amount will be. Do you have a high deductible plan? If so, even though it's "covered" it could just apply to your deductible and you will still see a very high dollar amount at the pharmacy.
If you don't find that out ahead of time you could go through this entire process only to find out at the pharmacy that it's still $1200 a month because of a deductible or it's a certain 'tier'. How frustrating!! and then you will probably get mad at the pharmacy because you went through all that trouble and it's still too much. (It's not their fault!)
Do you have a coupon? Do you qualify? The Wegovy website has a coupon or payment assistance plan that you can sign up for that can take a certain dollar amount off your copay. READ THE FINE PRINT! If you have an insurance plan that is funded by the government such as Medicaid, Medicare or Tricare, you are not allowed (by law) to use a coupon if it's billed to that plan. They also can rescind or modify the offer at any time, so if it doesn't work for you don't @ me.

I have a 6 (almost 7) year old running fever.
She weighs 47.5 pounds as of her dr appointment last week and now I am looking at the Tylenol and Motrin dosages that say 10mL for 6-8 year olds who weigh 48 pounds but 7.5mL for 4-5 year olds who weigh 36-47 pounds.
The jump from 7.5mL to 10mL seems substantial and she’s ‘not quite’ 48 pounds so I considered giving between the two doses and then decided to just ask and see if I get a better answer.
What is the correct dosage? I don’t want to give her too much but want to give her what she needs in terms of comfort.
(Our pharmacy is closed for the holiday, as is her pediatricians office until morning).

Hi. First time posting to this community but I have a question for Walmart employees, specifically those who work at the service desk/customer service. I had a return of two things. Both clothing. My husband bought them with our debit card, running it as a debit, not credit. We returned them today hoping to get it put on a gift card so we could have a bit extra to spend on food. The lady that did the return started out friendly, but when I asked if she could return it to a gift card, mentioning I had done many returns, with a receipt everytime and they gave me a gift card, her tone shifted and she started being a bit smart ass with her words. Sorry a bit of tl;dr there, but are employees able to return to a gift card from a debit purchase? She claimed that she "was informed that she couldn't" when I asked.
In spirit of treating others as I'd want to be treated, we went through with the return and I was civil with her, but I wasn't happy she didn't try to ask if it was something she could or couldn't do, given that it's never been a problem before, especially returning clothing to buy a size up or down. Many of the customer service workers have been very pleasant and nice to be around.
Also, I read through the return policy and nothing strictly says it HAS to be returned to the debit card only. It says cash can be provided. So it's really left me with questions.

I’ve called over 160 pharmacies in the Illinois area (and some close to IL in Wisconsin and Indiana) and everyone’s out of stock of the 1mg and saying they don’t know when they’ll have it - my dr won’t put me on 1.7 until I’ve been on 1 for 4 weeks. Can’t find 0.5 either. If anyone has any leads in the area, I beg you, please message me!
